Patient Records Requests
At any time, a current or former patient of Complete Wellness may request access to their own medical records (protected health information (PHI)). You have the right to inspect or obtain a copy, or both, of the PHI. You also have the right to request that your PHI be transmitted to a designated person or entity of the individual's choice.
You have a right to access this PHI for as long as the information is maintained by Complete Wellness.

To obtain a copy of your PHI or to have it directed to a specific agency of your choosing, there are 2 setps.

If you want your records sent to an agency or clinic, we need your permission in this Release of Information Form.
