Interested in being a new patient?
We've made it really simply to begin the process. Here are just a few steps to start:
1. We need to know who you are. Select this button and complete all the information and hit submit at the end. You will be prompted to upload a picture of the front of your insurance card and back. This is important if you wish to use insurance to pay for your services.
2. All health care practices are required by law to provide you a "Good Faith Estimate" of the cost of your services. Please review the information here.
3. In order for a parent to discussing scheduling and payments of a patient 12 years old or older with our front office transfer, we must have permission of the patient. There are other situations in which a patient might need a third party to discuss their case with our office. In these cases, this form must be completed for each third party contact.
If working on the computer isn't for you, the documents can be printed from the displayed form. Once you've completed the form, you can email it to office@CompleteW.com.

We can even verify your insurance before your first visit.
